Place of Birth

In this section you must give details about your place of birth and nationality.

Other Names

You should enter any other names used at any time during your lifetime and the dates during which the names were used.

Enter each forename, middle name(s) and surname separately using the Add Name button.

You should also add any aliases you have used e.g. William known as Bill.

Not giving these additional names may delay your application.

Changed Surname

If you have changed your surname, you must provide your birth surname and the year you changed it. Where names have changed they should run in date order with no gaps.

If you entered the title 'Ms' at the start of the application,, this question will default to Yes and you will be required by the DBS to enter a surname. If you have never changed your surname, simply repeat your birth surname again in this section.
