Use this form to:

  •  Update the Senior Leader listed on your account

This is often someone in a senior position for example: Trustee, CEO, Pastor, Elder, Manager etc

Use this form to:

  • Change the organisation name
  • Change the organisation postal address
  • Change the Recruiter surname
  • Change the Recruiter email address

Important information to stop your form from being returned

  • We cannot accept typed signatures, please either provide original, scanned or electronic signatures only.
  • The same email address cannot be used for more than one person or for more than one organisation.
  • A contact phone number must always be provided.
  • New Recruiter Appointment Forms and Senior Leader Appointment Form must be signed by current Senior Leader and Current Lead Recruiter - unless one of them has left the organisation. 
  • If Senior Leader has left or we have never been previously advised of one this must be updated before or at the same time as any Recruiter changes.
