Getting the form ready to send to thirtyone:eight

You should take a photocopy of the form and store this in a secure place so that it can be compared against the DBS certificate for accuracy.

Validation Form

The DBS application form(s) should be sent to thirtyone:eight with a completed ‘Validation Form’. This form has sections for both the Recruiter and the applicant to complete

Please keep a copy of the Validation Form for your own records

(Remember to send us the original, not the copy). You can use it to check against our direct debit notification and the details also enable you to track the progress of the disclosure application online on the DBS website.

When you are satisfied that the forms have been fully and correctly completed, place them and their corresponding Validation Forms into an envelope and send them to:
